MCT Ketogenic Diet Epilepsy: What This Review Found

Summary

What was studied

This systematic review examined medium-chain triglyceride (MCT)-based interventions for people with drug-resistant epilepsy. The interventions included the MCT ketogenic diet and MCT supplements used with or without a ketogenic diet.

Researchers searched four medical databases from inception through April 2025. They included 12 studies: three randomized controlled trials and nine observational studies.

What they found

One pediatric trial found no clear difference in seizure outcomes between the MCT ketogenic diet and the classical ketogenic diet, even though the classical diet produced higher ketone levels. However, equivalence between the diets was not established.

A small pediatric crossover trial suggested possible benefit from a ketogenic diet enriched with C10. A small adult trial also indicated a possible therapeutic signal from MCT supplementation. Observational studies generally reported improvement from baseline but had methodological weaknesses. Gastrointestinal side effects were common but often manageable.

Limits of the evidence

The certainty of the evidence was low to very low. Studies differed substantially, so the findings were synthesized without a meta-analysis. The pediatric crossover trial and adult randomized trial were small, and the observational studies had methodological weaknesses. It remains uncertain whether the MCT ketogenic diet is equivalent to the classical ketogenic diet, and evidence for MCT supplements without a strict ketogenic diet is very uncertain.

For families and caregivers

MCT-based interventions may offer a flexible dietary option for people with drug-resistant epilepsy. Low-certainty evidence suggests that the MCT ketogenic diet may produce seizure outcomes similar to the classical ketogenic diet, but equivalence has not been proven. Gastrointestinal side effects may occur and are often manageable. Evidence for MCT supplements without a strict ketogenic diet remains very uncertain.

What to watch next

Larger, well-designed studies are needed to clarify whether the MCT ketogenic diet is equivalent to the classical ketogenic diet and whether MCT supplementation without a strict ketogenic diet is effective, tolerable, safe, and sustainable.
