CBD May Better Prevent Seizures Than Stop Them

Researchers studied cannabidiol (CBD) in a lab rat model of malformation of cortical development (MCD), a brain development problem linked to hard-to-treat epilepsy. They used Wistar rats with an induced cortical malformation called microgyria and compared them with rats without the malformation.

They tested brain slices from two age groups: juvenile rats and adolescent rats. In these slices, they triggered seizure-like activity and looked at whether CBD changed that activity depending on when it was given. They also measured genes related to the brain's endocannabinoid system to examine whether those changes were associated with CBD responsiveness.

What they found

CBD had different effects depending on age, brain condition, and timing. When CBD was given at the same time as the seizure-triggering solution, it reduced some shorter seizure-like events in rats without MCD and reduced very long seizure-like events mainly in rats with MCD, in both age groups.

CBD did not reduce seizure-like activity once that activity was already established. But when slices were exposed to CBD before seizure-like activity was triggered, CBD delayed the start of events, lowered the total number of events, especially in juvenile rats without MCD, and abolished very long-lasting events in slices from adolescent animals.

The researchers also found age-related differences in genes tied to the endocannabinoid system in rats with MCD. Juvenile MCD rats had higher expression of several related genes, while one gene was lower in adolescence. The authors suggest these developmental and molecular differences may influence how CBD works.

Limits of the evidence

This was a preclinical lab study in rat brain slices, not a clinical trial in children or adults. Brain-slice results do not always predict what will happen in a living person.

The study looked at seizure-like activity created in the lab, not real-world seizure control, daily functioning, side effects, or long-term outcomes. It also used an experimental model of one type of cortical malformation, so the findings may not apply to all MCDs or all epilepsies.

Because the effects changed with timing and age, the results are complex and do not show that CBD will help every patient with MCD-related epilepsy. The gene findings show associations, but they do not prove why CBD worked differently across groups.

For families and caregivers

This study suggests that CBD may affect seizure-related brain activity differently depending on developmental stage, cortical organization, and whether it is used before or after seizure-like activity starts. In this lab model, CBD appeared to have stronger preventive effects than effects on activity that was already underway.

For families, this is an early research finding, so it should be seen as a clue for future studies rather than proof about how CBD will work in a specific child.

What to watch next

Stronger evidence would come from studies in living animals and clinical studies in children with different types of cortical malformations to see whether age and timing of CBD treatment are linked to seizure outcomes.
