Fertility Treatment Usually Safe For Women With Epilepsy

What was studied
This paper was a systematic review about infertility treatment in women with epilepsy. The authors searched several medical databases through March 2025 and looked for reports of women who had fertility treatment, such as in vitro fertilization (IVF), ovulation induction, or hormone treatment.
Out of 1,216 publications found, only 4 studies met the review rules. These 4 studies included 16 women ages 25 to 46 who underwent infertility treatment. All but one had epilepsy. The studies were small and included case reports, a case series, and one cohort study.
What they found
Overall, seizure control was generally stable during fertility treatment. Most women stayed close to their usual seizure pattern.
Seizure worsening was uncommon, but it was reported in some cases. These events were primarily reported in association with hormone treatment and lower antiseizure medicine levels, especially lower lamotrigine levels. Reported events included breakthrough seizures when lamotrigine levels decreased, seizure clusters associated with follitropin beta, and one new-onset seizure following dehydroepiandrosterone exposure.
The review also reported that multiple assisted reproduction attempts resulted in live births with different antiseizure medicine regimens, as well as in some patients not receiving antiseizure medicines.
Limits of the evidence
The evidence is very limited. Only 4 studies were found, and together they included just 16 women. Most of the evidence came from case reports and a case series, which are useful for signals but cannot show how common a problem is or prove cause and effect.
Because the studies were small and the treatments and medicines varied, the review cannot tell which fertility treatments are safest, which antiseizure medicines carry more risk, or how often seizures change during treatment. It also cannot prove that the hormone treatments directly caused the seizure changes.
For families and caregivers
This review suggests that fertility treatment may be feasible for many women with epilepsy without major seizure worsening. That is somewhat reassuring, but the evidence is still limited.
For families, the main takeaway is that hormone treatment may affect seizure risk or lower levels of certain seizure medicines, especially lamotrigine. This means careful follow-up during fertility treatment may be important, but this review does not provide firm rules for care.
What to watch next
Larger prospective studies are needed to better define antiseizure medicine-specific effects and to track seizure control and medicine levels during fertility treatment.
