What was studied

Researchers reviewed records from 93 children with febrile status epilepticus admitted to one hospital between January 2020 and April 2023. All received an initial benzodiazepine within 80 minutes of seizure onset.

Children were grouped by whether the first benzodiazepine was given within 40 minutes or between 41 and 80 minutes. Researchers assessed impaired consciousness at 6 hours, use of anesthetic coma therapy, acute encephalopathy, and neurological sequelae.

What they found

Overall, 33 children (35.4%) had impaired consciousness at 6 hours, and 16 (17.2%) required anesthetic coma therapy. Neurological sequelae occurred in seven children (7.5%), including one death.

Anesthetic coma therapy was used more often in the later-treatment group than in the earlier-treatment group: 22.7% versus 3.7%. Its use also increased as the delay to benzodiazepine treatment became longer. The groups did not differ significantly in rates of acute encephalopathy or neurological sequelae.

Limits of the evidence

This retrospective study identified an association but cannot establish that delayed benzodiazepine treatment caused the greater use of anesthetic coma therapy. The study included 93 children at a single hospital and excluded those who received benzodiazepines more than 80 minutes after seizure onset. The abstract does not report long-term outcomes or provide details about adjustment for potential confounding factors.

For families and caregivers

In this study, later benzodiazepine treatment for febrile status epilepticus was associated with more frequent use of intensive treatment with an anesthetic coma. However, the study did not find statistically significant differences in acute encephalopathy or neurological sequelae, and it cannot show that treatment timing caused the observed difference.

What to watch next

Larger, multicenter studies could help determine whether the association between benzodiazepine timing and the need for anesthetic coma therapy is consistent and clarify effects on neurological outcomes.
